Job Description

The Medical Coder analyzes clinical documentation and accurately
assigns diagnosis codes to maximize reimbursements for the organization. The employee must
be able to function with a high degree of independence and exercise judgment and discretion.

Responsibilities
  • Review patient’s medical history and physical, perform medication review, and recommend patient care plans.
  • Review documentation and accurately apply diagnosis codes using current ICD coding guidelines.
  • Problem solve and troubleshoot documentation for coding and OASIS discrepancies, logical inconsistencies, errors, and completeness.
  • Collaborate with clinicians and billing staff to resolve diagnosis coding and OASIS issues.
  • Maintain knowledge of OASIS, coding, and clinical practice standards to ensure current knowledge of procedures and national standards.
  • Conduct clinical record audits and provide feedback related to documentation.
  • Identify educational and corrective opportunities for clinical staff. Communicate findings to clinicians and agency management.
  • Meet productivity and quality standards.
  • Work extensively with protected health information and adhere to HIPAA privacy and security regulations and policies.
  • All other duties as assigned.

Qualifications
  • US RN, NCLEX - Open to PHRN license holders or non PHRN license holders
  • Previous home health and hospice coding experience highly desirable.
  • Knowledge of anatomy, physiology, disease processes, and medical terminology. Familiar with operative terms and pharmacology.
  • Hold or be willing to obtain Certified Coding Specialist (CCS), Home Care Coding Specialist-Diagnosis (HCS-D), Home Care Coding Specialist-Hospice (HCD-H), Home Care Clinical Specialist – OASIS (HCS-O), and/or Certificate for OASIS Specialist-Clinical (COS-C) by fulfilling continuing education requirements.
  • Proficiency with the use of a computer and a variety of computer software and applications.
  • Applies professional knowledge and uses critical thinking skills.
  • English language proficiency.
  • High attention to detail and excellent organizational skills

  • Must be willing to work in Outsourced - Zeta Tower ( 7th Floor, Zeta Tower, Bridgetowne, Quezon City 1800
  • Must be willing to work on a grave yard/ night shift schedule / flexible schedule
